Sony VCR SLV-920HF Tape getting pulled out

I keep having this problem even after i cleaned the heads. The cassette will not play or even rewind. I put a cassette in and it starts for a couple seconds and then stops. Even if i try to FF or RW it will not move. When i eject the cassette some of the tape has been pulled out of the case. Only about 2-3 inches of the tape is pulled out. It never breaks the tape or makes in unuseable. This used to happen every once in a while but now it happens every time.
